OK I have a theory. It seems to me that Jan once said that Shaun has so many friends and relatives around him. If he didn't he would turn to her. Maybe Marlena is treating her and has gone into her mind (like she said she could do with criminals) and this is all Marlena's visualizing what it is like to be a killer. Jan is really the serial killer.
