Chores Template?

Does anyone have a chores template? We are going to Irleland over Christmas and in preperation and to make some spending money for our trip DH & I told DSD that we would pay her for her chores and if she received a certain grades on her tests and projects. I am trying to put a spread sheet together to keep track for/ with her and was wondering if anyone had a template that had worked for them?

In the past I have just printed up a chore chart on Excel. This I could costumize it for each child.

It might be just as easy to give her tokens for each item she does. Then you can 'cash out' the tokens... when it gets closer to your trip.
