When presenting a magic band for payment at a restaurant, using the dining plan, has anyone ever been asked to present an ID as well?

No, but you do need to know the PIN code associated with the band.

Yes. But we usually add the CM discount, DVC discount, TIW discount, etc.

If you are using a discount like TIW or AP, you will need your ID. You may also need it if you order alcohol - some places may ask to see your ID or passport if you are from outside the US.
