Car Rental through Costco with Budget

I have rented cars at MCO through Costco with Budget for many years. Nine months ago I reserved a van (12/5 through 12/14) for nine days for a total of $366.00 showing on my Costco membership reservation. Two days before I am to arrive in Orlando, I get an email reminding me of my reservation and it would be $592.00. I phoned Budget and they said that it was correct and after some investigation, they informed me that on my profile I had stated that I wanted insurance etc., which I had never asked for or requested it on my profile. This has never happened before and was very lucky to get a new prepaid reservation for $10 extra dollars.
