I only had two pre-booked... odd...
60 days out all 3 have to be for the same park. If less than 3 are booked, the remaining FPs can be booked for any other park and any Tier.Didn't work for us that way, maybe glitch. Even booking 60 days out, I had to make all 3 in same park.Oh well, maybe next time!
Farro, booking the third in a different park can be done on the day of, but not in advance. Once you tap in for the first of the day, I think that opens up the option for booking your third in a different park. (This, of course, does not explain MickeyMinnieMom's problem. Maybe James Cameron has hacked her account?)

Not in my experience.The 3rd can be in a different park as FP booking has been optimized for park hopping.

How long ago did you notice this? It’s been working for others who follow the steps for park hopping in the Addendum.Not in my experience.

Thank you for reporting in!I can verify being able to book FP+ in another park on the same day if you have booked less than 3FP+ in the first park
Yesterday I had 2FP+ booked at Hollywood Studio - our intention was always to go to a different park in the evening, so I left the 3rd FP+ open when planning
Used the first FP+ at ToT
Had a 2nd FP+ booked for TSMM - however, after visiting Tune-In Lounge we decided to skip since we had ridden it twice at park opening
On the way back to the resort I booked a FP+ for FEA at Epcot for later at night (thank you whoever dropped that)
Then I cancelled the TSMM I had booked since the time window was still open
Then added another FP+ for a Tier 2 ride at Epcot
