Calling for ADR's question

Say I'm arriving 12/1/12 staying till 12/5/12....do I have to call 6/4/12 for 12/1, then 6/5 for 12/2, 6/6 for 12/3...etc??

Or can I make all my reservations for my entire trip on the first ADR day?

You can make all your ADR's the same day

If you are staying at a Disney resort, you can make all of your ADRs for your trip (for up to 10 days) at your 180 day mark (it's referred to as 180 days+10). If you are staying off property, then yes, you have to call each day to make your ADRs to make them at exactly 180 days.
