Yes, you do need one. Don't give up on it; keep checking. I just got one today for a party of four for this coming Sunday. There seems to be much more turnover in ADR availability close to dates

Yes, for all meals there, same like other ADRs.

I was unable to get reservations for BOG online, but when I called and spoke to a CM they were able to find a great time slot on their system. Not sure why that is, but I always call to check openings besides using the website.
