Was the baby not properly strapped in the carseat? How could a baby have head injuries unless he actually fell out of the carseat when it fell from the cart?
If the handle is not up--the baby is vulnerable if the car seat falls face down. I could have sworn in some type of child safety class that it was recommended to leave the handle up, just in case. Although, the preference is certainly to not place the baby where it can fall. But for some reason, keeping that arm handle up sticks in my mind as a prudent choice for "just in case".
I shop at Publix (I love Publix) and they push the cart out for you and load your groceries. This is part of their "service." If my son is in the seat, he gets pushed by the employee. They usually are very friendly and strike up conversations with him. We go there a lot and know most of the employees.
I agree with some of the other posters. I was never comfortable putting the bucket car seat in the little seat area. I always put it in the main cart area. It took up a lot of room, but you just put stuff underneath the cart or in the seat.
This was a tragic accident. I feel for everyone involved!
However, some handles must be left down when in use in the car. Or at least that's how it was back when I needed an infant seat (many years ago) so maybe things have changed.

Actually depending on how it fell it may have been better for the child to not have been strapped in.

It sounds like the child was strapped into the carseat, but the carseat itself fell out of the cart, with the child in it.
