Would you write a letter of recommendation?

I would see if you can look in her HR file, usually supervisors can have access to this info. You would be able to see her past performance reviews, attendance record etc., and maybe it would help to be able to write a bit more of a personal letter. You may even be able to quote her former boss if she had some favorable things to say in her reviews.

we had the same policy where i supervised. i always suggested people keep a copy of their most recent employee evaluation and upon being asked for references from their current or most recent employer just explain that we had a no letter policy and offer the eval (and since we did an exit evaluation on everyone, if they left it would have reflected under what terms and if they were eligble to re-hire).
