I try to give the server the benefit of the doubt, as I am one myself. But, if you did not see your server for a full hour, something was definitely wrong. You were told she was "on break?" I think I would have point blank asked the manager where she was and what she was doing. I can't imagine a waitress going "on break" when she's only been working an hour. Where I work we get breaks when we're not busy, but even then if we have tables we still take care of them, we don't leave them to someone else. We never get a real, uninterrupted break. We are always hopping up and down to give refills, dessert, etc.
Maybe something was seriously wrong, and that's why you didn't see the server and they were trying to "cover" for her. I don't know.
I wouldn't leave a tip though, since you weren't adequately taken care of.
And I probably wouldn't be in any hurry to go back to that restaurant.
