Where are the FP machines in DL & DCA

Is there a webpage that shows all the FP machine/kiosk locations in both the parks?

Unfortunately no. The closest thing is the map of the parks that lets you know which rides take Fastpasses.

The FP machines are usually located right next to the ride. There is always a CM milling about so you can ask if you can't find it. I usually have to ask once a trip

Mostly, just go near the ride entrance and look or ask...

A couple of notable exceptions:

Star Tours FP distribution is across the way outside of Buzz
Radiator Springs Racers FP Distribution isn't even in Cars Land... it's on the main thoroughfare between the entrance to Cars and Bugs lands.
Tower of Terror FP Distribution is across the street

I think that RSR is probably the most unusual... The first time we went to grab a FP, we walked all the way down into Cars Land and were sent back up.

They didn't intend it to be this way when they first designed Carsland. I think the place they sell the RSR ride photo's is the area where they designed FP to be for RSR. But they knew the popularity would make this an issue. So they decided to "temporarily" use the old Bug Life 3-D FP machines and of course just continued to use them.

As well, the Disneyland App on your phone also has the locations, under guest services, however there doesn't seem to be a way to filter only fast pass locations on the app.

Yes, you can filter it.
it's not too apparent, you have to use list view, the select Fast pass (or other item you are interested in) and it will display information on it. There is also an icon to show the locations in that info page. Tapping on the pin drop icon will just show the Fastpass locations. This is great, now I do not need to print out the map.

Thanks for puzzling out the app. I will definitely be using the app to find the fast passes in December.
