We are staying at AKL in December and wondered when and how we request a crib for our room?

I did on the booking day, and plan to call 5 days prior my check in to ask for a specific building and for the crib again, I booked online and it did gave me the choice to add the crib as a request

I've always seen it as an option when checking in online. You could always call Disney anytime after you have booked and request it.

I'm pretty sure the Disney provided crib is a pack n play. If your child is a small baby, that would probably suffice. However, I'm fairly confident that my 18 month old would not be comfortable in one of those for many nights in a row. I rented a crib from a 3rd party and it worked out great.

I'm not totally certain how it is at AKL but at other deluxe resorts there is only a limited supply of actual cribs. You could get one, but most folks will get a pack-n-play.

You can call at any time and add it to your reservation.

In my experience, don't be too alarmed if it's not in the room when you arrive or otherwise not brought to you without you having to give them a friendly reminder nudge. I would recomend either calling housekeeping from your room (don't call the front desk) or physically ask at the front desk.

I agree with @KrazeeK120 most often a" crib" is a Pak n Play At Disney resorts. Resorts have a limited supply, if any. If you need a real crib you need to rent from an off site vendor like ABBF.

My thoughts exactly, it will more than likely be a pack n play. If you definitely need a crib, I would call and rent one to be delivered to your hotel.
