I would just say hi. Usually it doesn't even come up, and they don't know I can't remember. If DH is with me, he will introduce himself and say, "Hi, I'm x. She never remembers to introduce me". I do the same for him.

I would just be very friendly and not worry too much. They know it takes a while, but usually you'll hear their name several times. Of course, if there is one you are closer to, it doesn't hurt to ask them who someone is.

This. There are a number of coworkers who have been in the same building as me for the last 5+ years but I can't keep their names straight. I just don't refer to them by name.![]()
If it's someone I run into on the street, I might say "I'm sorry, I'm terrible with names, yours is... ?"
true story:
My dad was an identical twin. He and my mother lived in Houston. His twin and his wife lived in Wichita Falls. One of their other brothers lived in Dallas.
My parents bought a new house and visited a nearby church. After the service, a woman came up and addressed my dad as Mr. last name. She asked how his brother in Dallas was doing (calling him by his first name.) She asked my dad if he remembered meeting her at his brother's church. At this time, my dad knew she had made a mistake. He told her, "I'm not who you think I am. You met my twin!"
They both laughed and our families began a lifelong friendship.
I usually say, "Oh hi! Have you met my husband (or another friend or anyone else who happens to be with me)?" At this point, they often introduce themselves and I get to hear his/her name again.![]()

It happens to me everyday. I work with a variety of military classes and spend days with the students. I can be in the grocery store or the movies or whatever and I hear "Mrs. Tina, Mrs. Tina or I hear Mrs. xxxxx"
I usually just smile and make normal small chitchat with them. I don't typically ask their name unless I was going to be working/seeing them on a regular basis.
You can politely say something like "My brain is still on summer vacation, what is your name again?" If someone just say hello and starts to chitchat but doesn't call you by name, say something like I'm so bad with names, I'm *insert your name* and that person will also say their name. After he/she tells you the name, repeat it back saying something like "Nice to see you again, Mary/Joe/Jane/Jim."
