what is the procedure for making park reservations?

I cannot find it in the stickies.
They weren't doing the park reservations on our last trip (pre-Covid).

How does this work? Do you call the main Disney line? Is this done through the app?

Can we stay off-site and still get reservations?

Disney website has all your answers under Park Pass System

If you have tickets linked to your account in MyDisneyExperience it gives you the option to make park reservations there. It's towards the top of your screen. Mine shows a picture of a family.

There's also an option to 'Learn More' which should answer other questions you might have.
