What info do you leave for babysitter?

I know this is mostly a common sense question but I could use some help.

DH and I need to leave DD#2 (she will be 1yr on Sat) with our niece for probably 10 hrs while we go to a wedding and I need to know what info to leave.

I'm already planning on leaving our cell phone #'s, the phone # for the reception site, her pediatrician's name and #.
I'll show her where the thermometer/medicine is with dosage instructions...

I don't know what else to leave....

We TOTALLY trust our niece (plus dh's sister is just 5-10 min away) but I'm lost.

Can you tell this is our first time leaving her with someone other than a grandparent? :rotfl:

Anyone have any suggestions?

When my boys were little like your DD, I would leave a list of their favorite movies, a list of a their nap schedule along with what they needed for their nap- blankets etc. Anything that would make them more comfortable, I figured the happier they were the better things would go for the babysitter.

I leave my girls' insurance information along with everything else mentioned above. God forbid anything would happen to them, but you just never know. We took them to the emergency room once after a car accident and the first thing they asked for was our insurance info. They took them right in afterwards.

I'm so paranoid about her having the phone #'s that I know I'm forgetting "normal" things.

I do a lot of childcare, and I always ask parents to give me a written consent to authorize medical care in the event of an emergency. I've never had to use it, but I think it's better to have it than need it. The wording on it is usually something like this: (I got it from a legal document for home childcare providers.)

"I affirm that I have legal custody of (child's name). I give my authorization and consent for (babysitter's name) to authorize necessary medical or dental care for this child. Such medical treatment shall be provided upon the advice of and supervision by any physician, surgeon, dentist, or other medical practioner licensed to practice in the United States." The form then has both parents' signatures and the date.

I also have the parents give me the child's full name, birthdate, doctor, doctor's phone number, medical insurance provider and policy number, and an alternative adult to contact in case I can't reach the parents.

It doesn't matter if it's hand written or typed.
 
I list the aprox. time my son eats each meal and snack. When he has naps. WHat foods he can not eat ie peanuts ect. All the phone numbers: ours, doctors, police, fire and neighbors. Plus special beadtime routine instructions. For us it is dinner at 6, bottle at 700, bath at 730 jammies and story at 745 put into crib at 8.

Is your niece familar with babies, I know that I had to show my dad how to diaper my son as it had been a while and he wasnt used to the diapers, not to mention he had only girls.

YOu might want to ask her what info she would like and let he know what she can/can not use in the house, the computer for instance.
