I do a combination of the above. My set is platinum, but the wedding band has channel set diamonds that were sauldered (sp?) in with nickel and the protective coating has since worn off underneath. Naturally, I am allergic to nickel, so I can't wear my wedding band for long periods without breaking out in a bloody rash. My dermatologist told me the only solution, really, was to get a new band, but he did give me some Elidel to use when I get really bad. My internist's nurse told me to paint the inside with clear nail polish -- haven't tried that one yet.
So right now, I wear my engagement ring and wedding band when I am going out, going to church, etc. However, I take it off to sleep, when I am hanging out at home, and when I work. I teach 3 days per week at a preschool and all that handwashing completely irritates the area and makes the rash 10x worse (not to mention the marquis-cut solitaire could double as a weapon and I am afraid I will scratch the heck out of some poor child with it). So I never wear it to work.

. Recently I've had some swelling/weight ((ahem)) issues and am only wearing the band. My wedding ring is screaming for me to go on a diet
