We went with Avery labels

We printed information on Avery labels and stuck them to the back of our DS's shirt. He was two. Here's what our labels said:

Hello!

If I am lost, please call my Mommy and Daddy.
They will be very worried about me
and will have their cell phones on! Thank you!

Mommy: <name, cellphone>
Daddy: <name, cellphone>

We already have our labels printed up for our trip in September. I love this idea. I also made an anklet for my dd (1 1/2) in case someone doesn't see the label, she will be "Labeled" twice. The boys (5,3,3) will know where their label is so they can tell a CM to call us.
