I never make a note of it. I too have diverticulitis. I tend to stay away from seeds/nuts/grains, especially if it's acting up. I can usually just look at the ingredients and avoid the offending object. However, I do sometimes ask them to remove nuts, especially if it's a salad, where they can get lost easily. I don't usually avoid strawberries much, except that I eat keto at home and fruits are basically sugar/crack for me. I would just ask when ordering. You might mention strawberries, since most people don't think of them as having seeds. I wouldn't bother with going the normal allergy route/noting it on your reservation, because you have to wait for the chef, they have to talk to you about the whole menu, and sometimes they only want you to order off the allergy menu, which can be very limiting.
