My European mother adopted the use of a top sheet when she got to the US, not just because it was the norm, but because she REALLY hated the hassle of wrestling the duvet in and out of the cover for washing. If you have a larger bed, maneuvering it in flat can be a hassle. I agree with Mom; I keep the top sheet and only change the duvet cover when it is visibly dirty or if I want to change out the color scheme of the room.
Someone here once said something like, wrestling the duvet into the cover was like, "Trying to stuff a wet noodle into a balloon."
One time I had such a pretty pattern on the top of my comforter, that instead of covering it with a duvet, I sewed coordinating ribbons at the corners, plus a few along the edges and in the middle, and tied a coordinating top sheet, (also with corresponding ribbons) to the underside. I folded the top over, so it looked like a top sheet was essentially, permanently attached. I just untied the sheet to toss in the laundry and only washed the comforter when necessary.
So, it truly is so simple a child could do it (DH knows the trick now too--DS was so sad when he figured it out
