USO book drive HELP!

We signed up for the book drive and shipped our books yesterday. I went to download the toolkit again (didn't print it last time) to get the verification form and I can't access it since the program is closed.

does anybody have the toolkit they can send me? I have the address I shipped the books too but that won't help.
