TSA pre-check married name

I will be going with my family to obtain TSA pre-check. I have no issue filling out the online form for anyone but my mom.

She was married, divorced, and remarried. It appears she only needs to bring her passport which has her current married name which also matches her license and SSN card. However, her birth certificate is obviously her maiden name. The online form confuses me (IDK if it is just me, or a confusing form) since it states a passport is sufficient, but then asks if your license name matches your birth certificate. In her case it does not, but it seems irrelevant because those documents will not be provided.

I tried calling the TSA and had been on hold over 30 minutes and gave up.

The answer for her would technically be No, but if she does not need those documents would we put yes? I do not want to fill it out incorrectly and delay or disqualify her for pre-check.

Any help or guidance is appreciated!
 
DL and BC are just a suggestion there as examples of ID documents. She's bringing a passport as her identity document. Her name matches what is on that document.
 
TSA (or whoever actually does it) is doing a background check on you. By giving them information such as previous last names it helps in that process of making sure that person is who they say they are and could avoid issues if there is someone with the same name who might not be eligible for pre-check.

Thanks! I will make sure we add the previous names in the prior section on the form to help with any of this.
 

I got it last November, just used my passport. They take a copy at the center and also your fingerprints.

I've lived at my current address less than 5 years, I had to give them my old address also, assuming for the background check.

Thanks again for all your help. I just completed the form and it confirmed "Legal Name must match exactly on all identification documents brought to enrollment" so the passport will suffice and I entered the previous names under maiden/previous so they have them as well.
 
Right. The reasoning is you had to provide all the back up documentation to obtain the Passport. Once you have it, it replaces having to bring it all again.
