Trip Insurance for Points/Airfare ?

Please help.....I noticed somewhere in these threads about getting some trip insurance that covers points and maybe airfare? Who do you get it from and how much does it cost. What is the coverage and what would they pay out.

When you make your reservation confirmation in the mail, there will be an insert with insurance information. When you call the insurance company they will give you the options and coverage for points, airfare, etc.

We usually get it.
