I always buy insurance. I never buy from DCL.
Mom has a heart condition so we buy insurance primarily to be covered in case there's need for medical evacuation. Having had friends who have required this we know all to well the outrageous cost associate with medical evac when outside the US. The peace of mind in paying for travel insurance is well worth it.
We use Allianz. It is who my TA recommends. She's the friend who needed evac when she had a stroke on a trip. She had Allianz and they handled everything beautifully. She's actually done some video testimonials for them about her experience.
I buy the Classic plan for us. They a basic plan as well.
If you have any preexisting medical conditions you need to buy within 14 days of making first deposit on your trip with Allianz. Other agencies may have slightly different timing, but its usually not much of a grace period from first deposit on the trip.
