If you have a 5 day PH and on the last day you decide stay one more day, can you go to Guest Services and purchase an additional day? thus resulting in a 6 day PH?
 
As long as it is within 7 days of first use, I believe you can.

However, you must upgrade to a Park Hopper Plus as there is not a 6-day park hopper.

I would go ahead and buy a 7-day PHP in the first place as days never expire and are good forever.

I tried to upgrade my pass and add a day to it on our last trip. I couldn't. I had purchased it a couple of months before our trip and she told me it had to be purchased within a certain number of days before our trip. I'm still not buying it actually, that doesn't really make any sense. I might have just had someone who wasn't very helpful.

Buy the biggest pass you'll think that you'll need. You can save the unused days for another trip.

If you have a 5 day hopper and all of the hopper days have been used, there is no value left to and you cannot "add" another day to it.

However, if you have a 5 day hopper and after using 1,2,3, or 4 days, you can get "credit" for the unused days towards the purchase of a new hopper (and at Disney, the smallest one that you can get is a 4 day hopper.)

Disney allows you to UPGRADE most park tickets to a longer Park Hopper ticket, an Annual Pass (AP) or a Premium Annual Pass (PAP) within 7 days of the first use of the ticket. They will take the entire price of the ticket you started using (assuming you purchased that ticket from Disney) and deduct it from the cost of the new ticket. The starting date of an AP/PAP will be back dated to the date you first used any admission off the old ticket. Disney will not allow you to upgrade Unlimited Park Hoppers (UPH) that came included in a Disney package plan such as Dream Maker nor will they allow you to upgrade certain promotional tickets not available to the general public. UPH's that were purchased at resort check in or added to a room only reservation can be upgraded. There are two very big caveats you must take into consideration when doing an upgrade. If you have used any plus options at all during that first 7 day window, Disney will only allow you to upgrade to a PAP. You will not be able to upgrade to a regular AP. This ONLY applies when you are trying to get a longer ticket during the first 7 days. The other thing to be careful of is that once the old ticket has been used up and the last day's admission has gone by, the ticket is not upgradeable even if the end of the 7 day window has not yet been reached (as in you had a 4 day PH and used it all and try to do the upgrade on the 7th day). There is one exception to this. Disney's Play 4 Days pass for Florida residents. Disney will allow that pass to be upgraded to a Seasonal or Annual Pass for about one month past the promotion's expiration date even if all four of the days have been used prior to doing the upgrade. But Disney will back date the Seasonal or Annual Pass to the date you first used the Play 4 Days ticket which may have been several months ago.

So that means if you have used 4 days on your 5-Day Park Hopper and want to upgrade the ticket to a 6-Day PHP, then you will only pay the difference between the 5-Day PH and the 6-Day PHP, if this is within 7 days of your first day of use.

But, be aware that in an upgrade you will be charged the higher "gate" price (not the Advance Purchase price) for the 6-day PHP.

And, if the 5-day pass was purchased from a vendor other than Disney, you will not get the entire price you paid in trade. Third-party vendors purchase tickets at wholesale prices and then add their own markup of 10-15%. Disney can scan the pass and determine the exact wholesale price for your pass. That is the amount you will receive in credit toward the full price 6-day PHP.
