I'll add...your doctor would need to make a statement for you that you would be disabled for 12 consecutive months with your disabling condition, you can't be on unemployment while you are filing for SSDI, the more regular your medical treatment the better chances of getting the benefit the first time/application. Because of your age, they would probably "diary" your case and, if you receive a clean bill of health down the road, your SSDI might end at a future date. Definitely get a rep to assist you with the application process. 
ETA: I should explain that the judge will consider that the acceptance of unemployment on the part of the applicant indicates that they are physically able to work and are looking for work while collecting unemployment payments....which will go against the claim of having a disabling condition.
