Survey tool to determine large party's interest in individual attractions?

I'm organizing a trip for two families and I'm considering making an online survey (like doodle or survey monkey) for everyone to rate the attractions so I can gauge interest before planning fast pass reservations and which parks to visit on which days.

But I don't want to reinvent the wheel. Does something like this already exist? Something that is easy for people to use and results in a spreadsheet or database of their answers?
 
How many people are going? I wouldn't plan for a large group to stay together the entire time. When we were planning for our recent trip, I made a sort of excel sheet with all the attractions by park, and asked my family (only the 4 of us) yes, no, or doesn't matter for each one.
 
I second the idea to totally nix the thought of all staying together.

I'd plan a meal a day together, and let each individual group plan their days.

Trust me, been there, done that, several times. Once I was the planner for a large group of family friends coming from Japan (I only came along as the planner, so while it was amazingly fun for me to actually be at Disney, it was WORK). Done trips with extended family (not really by choice), and it is impossible to please everyone.

Some folks are up at dawn and ready to go. Some folks will take their sweet time. And you'll be waiting on them.

Just plan a meal or something, and not individual attractions.

I do this for Food & Wine booths/events for our party of 5. I'm a big pivottable fan, so I use this to help rank booths for us to visit afterwards. Would work the same way with attractions.

Depending on the size of the group -- and people in it!! -- I also wouldn't plan on being together the whole time. Just my two cents. :)

I really like it - I will say this wouldn't work with my family because it is their first time at WDW and they have said to me to book what I would recommend/like from my experience and they will join. So, be mindful that you may have lower participation rates if your audience hasn't been or isn't familiar with the attractions.
