I heard that you could sub out the dessert at like counter service restaurants for other things like grapes or water, is that true? what are the options?
Most of the counter service places will let you swap out the dessert part of the CS meal for some type of fruit or a bottle of water. I've also been able to get a bag of chips (it was a resort food court, but I don't remember which). You have to ask at each place what your options are and it will vary from location-to-location and possibly from CM-to-CM. The third party venues (like the ones at DTD) are more strict about the DDP entitlements so those places might not have as many options.
Substitutions will vary from place to place so you'll have to ask at each venue. Most common are a bottle of water or a side from the kid's menu (e.g. grapes, fruit slices, carrot/veggie sticks).

we do this sub on our counter service all the time- it is hard to eat dessert everyday... I can't believe I am saying that! We always get the fruit options or water- We save the fruit for breakfast or a snack later... it really helps maximize eating--
For Kid options- I have also been able to ask to sub the chips for the salad... etc... just ask- if you can't they will let you know..
