sorry,another visa question

I am travelling to Orlando on a visa (hopefully!) with my 13 year old son who will have a passport. What happens on the plane? do I still have to fill in any forms for my son?
 
Yes you do,and you need a form too.

Green forms for VWP;white forms wihout- everybody needs one or the other
 

Actually you'll need to fill out BOTH types of forms! The green from is the Visa Waiver form and EVERY person has to have one of these, whatever age. I think it says on there that an adult can sign for a 'minor' (My DH signed our daughters last year when she was 4). The white form is the customs form (where you state you're not carrying any fruit etc) and one of these is needed for each family NOT individual! However anyone who has a different family surname needs a seperate form. In other words if mum and dad aren't married and the kids have dad's sruname, Dad and the kids go on one white from and Mum will need to fill out a seperate one!
