someone personalize Patnl34 autograph book cover with my name?

I have found the perfect cover but of course it is from someone not desinging anymore. Can someone personalize the below with my name Leslie? I also included her blank one. I just dont know how to make my name like she did. I dont want it changed in any other way so I hope someone can help me.

First, you should know I am not a designer and only have Word/PP at my disposal. Second, I couldn't find the same fill in a high resolution. If this isn't what you want or if it doesn't look right, I would make this request on a designer's thread who is willing to modify existing designs.
