Simple Sample Letter For School

Can anyone share a simple sample letter that lets the teachers and school know that I will be taking the kiddos out of school? The school requires notice, in writting, to each teacher, principal and school secretary (for keeping track of the absense). I am not talking about the one that lists the educational benefits of WDW. I am looking for one that can ask, nicely, for the best way to make up the work, etc.
