At a lot of these restaurants there are sides.....

French Fries, Mashed Potatoes, Chili, etc.

How do the restaurants handle these? Do you pay separately?
 
If the sides are priced separately, yes, you pay cash for them. If the sides are included with the entree, no. (In some cases, depending on the side, it may qualify as your appetizer.)
 
At a TS:
The Menus will list what is included as an appetizer and entree. There is also usually a section labeled as side items. If it's labeled as a side item, then you will pay out of pocket (OOP). At the end of the meal, you'll get two receipts, 1 for the food included in the DDP, and another for the OOP items, such as liquor and side items. The 18% gratuity will also be added to the OOP reciept.

An example could be at one of the steakhouses where mashed potatoes is included with one of the steaks, but marinated mushrooms is listed as a side item. You you pay OOP a few dollars for the mushrooms

At a CS:
Most times the meals are a combo that includes fries or some side. You can get the combo along with a dessert and drink. Again, any extra fries or anything not included with the combo will have to be payed OOP.
