If you have a room only reservation, is there an issue having an extra child on the reservation who may or may not come? We may have my nephew (under 17 so no extra adult fee) join us for our 2 days at Disney and I’d rather keep the room for 5 now, then cancel him and then hope to add him when we know for sure in a month. Are there any penalties or downsides to adding him now if he ends up not being able to come?

No need to cancel him until you know for sure if he’s coming since he won’t generate an extra fee. When my daughter came for part of my stay it was easier to just leave her on for entire reservation. I think she was only there for the first 2 days, we were in Universal before WDW. Then she had to leave for start of school.

No penalties. They don't check who is there. They don't care. This is another example of how a room-only ressie allows for more flexibility than a package.
