Can you bring a rolling backpack into the parks?
https://disneyworld.disney.go.com/faq/parks/restricted-personal-items/You may not bring the following types of items into the theme parks:
Suitcases, coolers or backpacks with or without wheels larger than 24" (61 cm) long x 15" (38 cm) wide x 18" (46 cm) high.
That says larger than 24" long x 15" wide x 18" high.
I wouldn't, but we saw several on our last few trips.
I wonder what on earth they do with those monsters when they want go on the rides!

I don't know what would piss me off more, folks disregarding the rule or Disney not enforceing it

I can't imagine why anyone would want to bring anything that big! What a PIA that would be.
I wouldn't do it but I'm sure there are people who have to lug around medical supplies that might. It's not very convenient to leave them at First Aid because then you to have to trudge all the way back there every time you need something.
