Yes. All guests, regardless of age or whether or not they will be ordering, should be included so you can be seated at a large enough table.

Yes, they need to be included in your count. They need a correct count to be sure to give you a large enough table and for occupation limits.

Yes! If the ADR is one that requires prepayment then you'll be asked how many guests are in each age group so that you won't be charged for the infant. The infant takes up space and counts towards fire codes so Disney does need to know that he'll be there.
