Yes, it is per person, unfortunately. When you're paying (they use gogoflight) read the options carefully.

Not really As long as you log off device one, the same user can log back in on device two. I have done this using my phone, Ipad and laptop. Only one device at a time so you have to make sure to log off.

I tried that a week ago with my iphone and ipad. It wouldn't let me log back in on my ipad.
