question about the visa forms?

Hi quick question,when we went to florida 2 years ago i remember filling out some forms with some details on including where we were staying.
Do you still need to fill these forms out???
If so we have a villa allocated on arrival so how can we put where we are staying on the from.

You still have to fill the waiver forms in.

We went with TCD and had an AOA villa with them. In the travel pack that they sent us with our tickets, this contained an address to use on the forms. Hope this helps

Everyone member of your family has to fill in a green Visa Waiver form and then you also have a white customs declaration form, just 1 for all the family.
I would get in touch with your tour op just before you go if they don't send you the info of address to use.
