Specifically the free dining ones:

- How often do they get sent out?
- Can you only get free dining if you are staying at a Disney resort? Or can you stay off site and still get free dining?

- Do Canadians ever get these codes, or only those within the US?

It is a mystery.

Can you only get free dining if you are staying at a Disney resort? Or can you stay off site and still get free dining?

Only people staying at a Disney Resort can get any Dining Plan. This applies to free dining as well.

Do Canadians ever get these codes, or only those within the US?

Canadians get PINs.
