Purchasing stock for minors

I am thinking about buying Disney stock from One Share for my kids. I started filling out the info, and it asks for a custodian name since they are minors, which it says will be listed with their name on the certificate. I really just want their own name on there by itself.

So, I back out and just fill it out with them listed as an adults. Nowhere throughout the rest of it does it ask for birthdays or any other information regarding their age. But, I'm scared to complete it like this. Will it hurt anything? Does it matter?

It's obviously not stock that's going to be traded, it's purely decorative memorabilia - but I'd hate to void the value of it by doing something wrong.
 
I'm not familiar with One Share but I bought Disney in my name as custodian for my daughter and then had it registered and shipped to my daughter only. I did it through the brokerage firm that I work for, so online brokerage may not be the same. Check it out though - maybe they'll let you do the same.
