Please help me find a cover for character dining book

I've searched through various DISigners threads and photo share sites to find a cover for character dining so I could make a request of having my two girls' names added. For some reason, I'm having no luck. I can't seem to find any out there. I'm sure I'm just missing them somehow. I've found pages to the characters to sign during the meals, but I'm trying to find possible covers for a 4 X 6 album. When I used the search forum, I found a really cute one with Mickey holding a platter & dish that said "We're dining with some real characters" (or something similar), but I noticed that thread hasn't had any activity since 2011, so I don't think that person is making them anymore. Can anyone direct me to where some covers for character dining books might be? Thanks for the help!
