You can go to a post office (that processes passports) ask for a form DS-11,
This is for a
new passport.
You cannot do a renewal as you were under the age of 16 for your last passport.
You will need a government photo ID, a 2x2 photo, certified long form birth certificate, marriage certificate...All originals. Also, a check made out to the Department of State for 110.00 for a routine 28 page passport book.
It cost more to expedite. Also, the post office charges a SEPERATE 25.00 fee, this cannot be added to the check being sent to the Department of State.
The form DS 11 has to be written out in black ink or typed in black ink.

Your Most Recent U.S. Passport:
Is undamaged and can be submitted with your application;
Was issued when you were age 16 or older;
Was issued within the last 15 years; and
Was issued in your current name or you can legally document your name change with original or certified copy of your marriage certificate or the government-issued document evidencing your legal name change.
If any of the above statements do not apply to you, you must Apply in
person.
