No, but it really doesn't matter. They will just ask you why you need one and they do that when you want to renew occasionally also. It won't make a difference either way.
Not sure what you mean by "record of previous accommodations" since the DAS isn't like the GAC with multiple options for accommodations? Isn't the DAS just an outside the queue return time? Are there more than one level of DAS?

DAS is for the ability to wait out of line, no other accommodations given nothing else to be recorded. think you are remembering the old GAC program where there other accommodations
There is only one accommodation that DAS does, and that is it allows you to get a return time for a ride and then come back and go through the LL line(old Fast Pass line). It does nothing else. If you need something other then that, you will have to speak to guest relations about it.

DAS is valid for 60 days, after which time you need to re-register. You may do that in advance by video chat or wait and do it in-person at Guest Relations.
Just remember that there's an advantage of advance DAS registration - you can get 2 DAS reservations per day for each park day (can be done between 30 days prior and 2 days prior to first park day).
Nope. That’s definitely odd. Did you create a new MDE account at any point? Strange for sure, but like others have said, it really doesn’t matter for getting a new DAS issued. I was treated like a new person requesting it for the first time after the new video registration option became possible, but they could still “see” that I’d had it in the past.
WDW or DL?
DL reportedly didn’t have a reliable way of tracking DAS visit to visit. WDW did though.
It’s also possible that it ‘dropped off’ because of time if you hadn’t been to a park in over a year
you will just need to re explain why you need a DAS then. It happens every now and then for me where I have to re explain to the CM why I need a das it is no big deal
