New sponsor form please

Hi, just wondering if anyone has a copy of the new sponsor form, would you be so kind as to email me a copy please.

I emailed the passport office last week prior to my visit at the weekend and received this reply.

"Thank you for your email concerning your upcoming visit to Disneyland® Resort Paris.

Further to your request, I regret to inform you that we do not have a copy of the current sponsorship form. However, this form may be obtained from the Annual Passport Office within Disneyland® Park or at the entrance of the Theme Parks. I sincerely apologise for any inconvenience this may cause."

Went to the Guest Services point at the park entrance and then to the passport office on Saturday - No forms available !!

Still I did get my 10% discount off the purchase of my Fantasy pass and my wife (Dream pass holder) had it credited to her account.

Included was the blackout dates for the Fantasy pass for 2009 & 2010

So if the person needing sponsorship has all the relevant details of the sponsors Annual Pass Disney should still give the 10% discount then?
