iTunes will try and update itself whenever there is a new version available. What it's doing is just adding little things, fixing bugs and the such like. You don't have to update if you don't want to (though sometimes the changes are big enough that not updating would mean you couldn't use the iTunes store for example) Updating should not effect your current playlists at all. To make sure an update is legitimate I always update by going through the "check for updates" thing on one of the pull down menus when I am already in iTunes.
OK, I got so excited that I ran over to the iTunes site and did not find a new version. Have they released the new version I've been waiting these long post-Vista months for or no?
Generally, there is no change to anything when it changes.

This is a good question. I'm almost positive you want the update, but it raises another question: Do you back up your Itunes library? I have over 6000 songs on my Ipod, but virtually every one came from CDs that I own.
However, if you had a large number of songs that exist only on your computer, you are courting disaster should your hard drive crash. I hope I haven't hi-jacked the thread, but I thought this topic fit with the concern of the OP(losing music in the process of an update)
