Need Ideas - Teacher Memory Book

Someone posted a thread over on the family board about end of year teacher gifts. A suggestion was made about doing a scrap/memory book for the teacher by sending home a sheet for each child to complete/decorate, etc. I think this is a great idea and would like to do this for dd's K and Spec Ed teacher.

The other poster added these 2 questions to each page for each child to complete:

My favorite thing about Mrs.Brown is ___________.
I remember when Mrs.Brown _______________.

Does anyone have any ideas for other questions that the kids can complete?

What about:

The thing I remember most from this year is:_____

Something I learned from (teacher) was:_____

You could also have a question for the parent(s). I am sure with Special Ed and K teachers parents would have lots to say. Maybe something like...

I am thankful for (teacher) for teaching my child ______

I am a teacher and I LOVE the idea of a memory scrapbook.
