Need HELP with my phone

The battery on my phone ran completly down :earseek:

Charged it back up. :earseek:

It keeps asking me for my PUK code, which has been the same code for years. It wont take it :earseek:

It is a Cingular phone. Help!

maybe it went back to the original setting....I know my cingular was preset at 12345 for the code...
