You can go to guest services on deck 3 once on board and ask for a button. I am pretty sure they are the generic ones where you can have your name put on it like they have at WDW. For example, "I'm Celebrating...then write in first cruise. They definitely have Happy Birthday buttons there too. It does not have to be your birthday at that particular time to get a birthday button.
They don't have a first cruiser button AFAIK. Here's what I was able to dig up. The buttons don't seem to have the same popularity as the ones in the parks, but that's likely because people are swimming and doing other activities where the button isn’t suitable. As sytrace mentioned, go to guest services and they will be able to assist you.
I ended up leaving my tooth brush at home and called up to Glen in concierge and told him my mistake. He told me he would take care of it and brought me a tooth brush right to our room...............he also handed me the "I lost a tooth" button you have a picture of above except he had taken a sharpie marker and added the word "brush" to the bottom of the button.
We all got a good laugh from it. Wish I had taken a picture of it.

DD lost a tooth at WDW a few years back. A cm at epcot saw it happen. She didn't get a button, but the cm arranged for us to watch illuminations from a private viewing spot! We also got magic glasses that turned every bright point of light into a Mickey head.
