If you stay at multiple disney resorts do they help move your luggage?

I am currently booked for 2 weeks at Polynesian in August 2014 but I am thinking to maybe split it over Polynesian, Floridian and Contemporary and go for 3 weeks

Yes, it's a very easy process and Disney will move your luggage for you :). When you check out of the first resort, take your luggage to bell services (or call and have them pick it up from your room). Tell them which resort you're moving to and they will take care of getting it there. Luggage usually arrives between 1-3 pm (could be later depending on how busy the resort is). Once your room is ready, you can either pick up your luggage from bell services or call from your room and have them deliver it. You would then do the same thing if you moved to a 3rd resort ;).

Do I keep the same KTTW card for the entire trip? I would assume so as I would have my park tickets and dining plan to still use during the transition from one resort to another.

You will get new KTTW cards with each resort stay. If you want the DDP for the entire trip, you have to buy park tickets with each stay (unless you have an AP, in which case you can book a ticketless package with room + DDP without the ticket requirement). Tickets are good for 14 days from first use, so getting just the maximum ticket days (which is 10), you would only be able to use those for 14 days before they're done. That would leave you with 7 days without tickets. If you didn't go with an AP, you could get 10 day tickets + DDP with the first reservation; room only for the second reservation and pay OOP for food; then get 7 day tickets + DDP with the 3rd reservation. Your DDP credits will be good until midnight of the day you check out, so you would need the DDP for each resort stay if you want the DDP the whole time. HTH :).
