Minor Authorization Form

I will be traveling with my children but without my husband on our upcoming cruise. Do I need to have my husband sign the minor authorization form? If so, does it need to be notarized? The form has a large blank area where a notarization stamp would easily fit but there is no mention of needing to have it done on the form or on DCL's website.....
 
I could be wrong but I think there is only an issue in the case of divorce. I travelled with my 15 year old daughter without my husband and there was never any issue. Of course, I do not know if there is an age issue... I guess I am not very helpful after all:confused3
 
Do yourself the favor and call Disney. The cast members answering the phone have got to be some of the most helpful people there are. The last thing you want is to get wrong or out of date information. Enjoy hte cruise, I can't wait until I can do one.
 
My husband and I often travel with our son without the other parent. I have been stopped going into Canada asking for the form, so I always travel with a form for that particular trip with a notary signature/stamp just in case.
 

The "rules" say you need the form signed and notarized if one parent is traveling with the kids alone. The "reality" is that it probably won't get enforced. I've only heard of parents being stopped when crossing the borders into Canada and Mexico. I travel with my custody papers that specifically state I am able to travel with my son any time, any where without permission from the father, but I have never needed them.
 
First I would call and check with DCL. I did cruised a 3 day Bahama with 2 minors(14&15) neither were mine in Dec 2012. For the Auth Docs, one doc had both parents sigs and the other had only one parent sigs. The notary was not required for my cruise. I got on the cruise without any issues. Also no issues when visting the Bahamas.
 
As long as you are listed on their birth certificate you are good To go. You only need the minor authorization form if they are not your children. I just contacted DCL about the same issue and it was clearly stated as long as I'm on the birth certificate we are good. They informed me that the regulations recently changed.
 
Is there a form on the disney cruise website? My son is bringing two friends. They are teens and I assume I will need something signed by each of their parents. I didn't see anything when I checked-in online.

It should have been in your online check in. Call DCL and they will send them to you to have filled out properly!!!

That's where my travel agent said it would be too. I guess I missed it. It didn't print when I printed the form that we have to sign and bring to the port. I will look again. This is my first cruise and I have no idea what I am doing.

It will all work out. Just try calling DCL. They will direct you in the right direction. Better be safe than sorry!!!!!

I'm taking my son and his dad and I are not together. To be SAFE I had his dad sign a memo stating when, where, how long, what ship, etc that he could sail with us. Just to be safe even though its not needed.

I have a 17 yr old and we are taking two of his friends who are the same age. I don't want to take any chances though, so I will give DCL a call about the forms. Our cruise is still 2 months out, but I don't want to leave anything for the last minute.
 
If they have passports, you will not be questioned. Because 'all' the legal parents have to apply for the passport (if sole custody, you have to show proof, if single parent only your name on BC), it is assumed 'all' the parents who need to know the child is traveling know.

For the unrelated minors, bring the permission forms, their medical cards, etc.

I cruised DC multiple times with my child and have never been asked for an authorization form. my name is on his birth certificate which I use for his identification.
 
I have taken 2 cruises with my sister and her 2 kids without their dad and she has never been asked for the form. The kids were 8 & 10 and 10 and 11 for the cruises.
 
I cruised last year without my DH and with my DS5 (California Coast with a stop in Ensenada). I had him fill out the authorization form. At the passport check on the return, the officer asked me "sole custody?", and I said "No, married but dad had to work, want to see the authorization form?". His response "You can show it to me for fun ..." ;)

As some have pointed out here..you very likely may not need it.

However, I would get is done and noterized, a simple thing to do, and have it just in case your asked for it. Standing at the check in counter, is not the time to have a discussion about whether you need it or not and not have it.


Plan ahead and be prepared, it could save alot of tears later.

I called DCL on this last week. My husband is currently deployed and will not be traveling with us. The DCL rep told me that the only need for this was if they child was NOT traveling with a legal guardian (i.e. parent). So if the child was traveling with a grandparent, friends family, etc. I'm not sure about divorced families if that is your case, you may want to call DCL.

If you are traveling with a child of whom you are not the parent or legal guardian, you must complete this form and bring it with you to the port terminal.

It's not part of on-line check-in. I looked too as we are bringing an unrelated minor. We'll make sure we have the forms long before we leave!!

ETA: Nevermind. I found it when I went back to edit documents. It was listed under optional forms.
