On my last visit my daughter bought us the military tickets and I was able to link them to MDE using the numbers on the back of each ticket. Then I made FP reservations 60 days out. We also had magic bands that the tickets were linked to. She bought the ticket on base then activated it at Disney but was able to link them prior to activation.
My sister bought Military Salute tickets for herself and for my DS and his family for our March trip. I linked them to MDE as @compshu illustrated above and had no problem making FP when our window opened.

Buy them from Shades of Green (over the phone). They will mail them to you for a $10 fee (FedEx) but they will send you already activated tickets that you can scan into the app. Bonus: don't have to waste time activating the tix when you arrive. Just go straight to the gate!

I admit that not having to activate tickets saves time & effort but sometimes it's worth it. When my family activated theirs they didn't have their MB with them (my fault - I didn't think they were needed to activate at DS on arrival night) so the CM gave them cards instead. Came in handy when both 8-year-olds lost their MBs in separate incidents several hours apart in Epcot one afternoon - didn't have to spend time at GR getting a card or time and $ buying replacement MBs.
Yes, at 60 if staying on property. I have recently done this for an upcoming trip and had no issues with booking FPs at the 60 day mark this month. We bought ours at the local ITR, though. We also did this same method a few years ago, didn't have an issue.
When the magic bands come in the mail (will get those free if staying on property), you have to link up those, too. I haven't tried the SOG ticket route of preordering tickets, but the response I've seen on here is that it comes activated.
