Lost DDE card

I think I accidentally threw away my DDE card. I still have the temporary in my wallet. Has anyone ever had to replace a lost card?

When I was getting things together the night before our trip in December, I went to where I thought our DDE card was and it wasn't there. I looked all over for it and couldn't find it. So, the next morning in the airport, I called the DDE office and explained the situation and they said they would have a temporary one ready for me when we arrived. When I picked it up, they said it would be $35 to get another permanent card, so we just went with the temporary one! We had no problems using it.
